Sadistic no-count killer Nanni Vitali and three other equally brutish hoodlums escape from prison. The foul foursome embark on a savage rape, murder, and robbery spree. Vitali even abducts and defiles frightened hapless lass Giuliana Caroli. Meanwhile, rugged police inspector Giulio Santini is determined to bag the despicable Vitali.

A gang of robbers takes possession of a load of diamonds.. Only one of them, Mark Lemmon, is arrested and sentenced to life imprisonment. But before being captured, the gangster had managed to hide the loot in a sewer. Five years later, Mackenzie, an insurer, pretending to be a mafia boss, assigns three petty criminals to arrange his escape. Once free, Lemmon finds the booty but now he has to deal with his new 'partners'.

Michael considers himself a genius criminal and plans the big coup. He forms three new gangs who know nothing about each other and each only know part of his plan. In the background Michael pulls the strings and mercilessly plays the actors on his stage against each other. This strategy also poses unsolvable problems for the police until something goes wrong and the house of cards collapses.

A drug deal goes bad and the heroin is stolen. One mob boss doesn't have his drugs and the other doesn't get his money. People are going to die until they discover who double-crossed them. Into this backdrop, Inspector Dario Mauri arrives from Milan to help clean-up Naples. His mission – find the drugs and stop the killing.

Inspector Betti (Maurizio Merli) is transferred to Naples and immediately after his arrival receives a warm welcome from The Commandante (Barry Sullivan), the city's crime lord. Betti then goes on a personal mission against corruption and organized crime, and tries to force the syndicate out of town with any means necessary.
